If you are adopted and do not know your biological family history, the family history section of the survey will be skipped. Your risk assessment will be calculated based on your personal health history alone.
If you do have information about your biological (blood) relatives, you can answer the family history questions to the best of your ability. If anything is unclear, please discuss it with your healthcare provider at your upcoming appointment.
