Please note that only the most recent 6-digit code sent to your email will work — older codes expire once a new one is issued.
Steps to login using the 6-digit code:
- Log out of the portal.
- Close the browser.
- Use the password-less option (6-digit code) to log in to the patient portal.
- Enter the latest 6-digit code from your inbox.
If you are still having trouble logging in with the 6-digit code, we recommend using the password option.
Steps to login using a password:
Log out of the patient portal.
Close your browser completely.
Reopen your browser and navigate back to the patient portal.

Click Use Password.

Select Forgot Password.

Enter your email address and click Reset Password.
Check your email and follow the instructions to create a new password.

Once your password has been created, return to the patient portal and log in using your email address and new password.
